In order to designate someone as a fiduciary agent (Attorney-in-Fact through the Powers of Attorney) we will need the following documents:

  1. A copy of the complete Power of Attorney documents (make sure they follow state guidelines). Please ensure there is a visible notary stamp or seal on the copy if required by your state.
  2. The notarized Fiduciary Agent Instruction Form completed by the Agent.

In order to designate someone as a fiduciary agent (Attorney-in-Fact through the Powers of Attorney) we will need the following:

  2. The specific information in this Information Document about the Cardholder and the person named as the Attorney-in-Fact.
