If your name has changed and you need to update your first, middle, or last name, or any combination of the three (Full Name Change)...

  1. Update your name with the Social Security Administration (SSA).
  2. Fill out and sign a W-9 form (Request for Taxpayer Identification Number and Certification Form) to match the name changes you want to make.
  3. Prepare a copy of a legal document (i.e. valid driver’s license, marriage certificate, divorce certificate, new Social Security card, etc.) that shows the updated name.

When you have the above ready to go, mail the following documents to Capital One, P.O. Box 60, St. Cloud, MN 56302:

  1. A written request signed and dated within the last 60 days.
  2. A copy of a legal document showing the name change (i.e. driver’s license, marriage certificate, divorce decree, SSN card, or court order).
  3. A signed W-9 form with your new name.

Once we receive your documents, it takes about 3 business days to process. Please call us to request a new debit card with the updated name once your account reflects the new name.

If you’re fixing a typo in your name on your Capital One deposit account(s) or changing your middle name to a middle initial (Partial Name Change)…

  1. Fill out and sign a W-9 form (Request for Taxpayer Identification Number and Certification Form) to match the name change you want to make.

When you have the above ready to go, mail the signed W-9 form to Capital One, P.O. Box 60, St. Cloud, MN 56302.

Once we receive your documents, it takes about 3 business days to process. Once you have requested a partial name change, please call us to request a new debit card with the updated name.
