Please note, your decision letter will be sent to the address associated with your Capital One account. If your address is not up to date, you can update your account information.

Customer Acknowledgement: To facilitate enrollment, Capital One has developed this form as a method of taking SCRA benefit requests from Servicemembers for their Capital One accounts. Capital One will use the information you provide on this form to verify your eligibility for SCRA benefits, including contacting the Department of Defense's Defense Manpower Data Center (DMDC).

Written requests should include a copy of active duty orders or a letter from the Servicemember's executive or commanding officer on official letterhead that includes:

  • Servicemember’s name and SSN
  • Active duty start date
  • Branch of service

 Please do not include Military or Government-issued IDs with your request 

Requests can be submitted at any Capital One branch.  The branch will forward your request for processing.

WHAT TO EXPECT NEXT

If you are eligible, we will apply benefits to each of your eligible accounts within 2 monthly billing cycles. There are different time frames for different account types, so you may see some of your accounts updated before others and you may receive multiple letters based on the eligibility of each of your accounts.

You probably received this letter because we weren’t able to verify your active duty status by looking you up on the Defense Manpower Database Center (DMDC) website or with the documentation you may have provided. To help us determine your eligibility we need you to send us a copy of active duty orders or a letter from your executive or commanding officer on official letterhead that includes:

    •    Servicemember’s name and Social Security Number
    •    Active duty start date
    •    Branch of service

Please refer to your decline letter for the specific reason your account was declined. SCRA requests are generally declined for one or more of the following reasons:

    •    The account was opened after the active duty start date
    •    The active duty period ended more than one year before benefits were requested
    •    The account was settled or paid in full before the active duty start date

Benefits are retroactive to your active duty start date or "order to" date for reservists and will remain on your account until one year after leaving or completing active duty. A letter will be sent to you at least 45 days before your benefits are scheduled to end.
