Acceptance Forms

InterestPlus Online Savings Account Acceptance Form

Provides Capital One the authority to maintain an account, certifies that the taxpayer identification number is correct and acts as the signature card for the account. This form cannot be used as an initial application for a new account or to add or update owners on an already existing account.

Corporate Certificate of Authority

Allows all listed individuals acting on behalf of the Business authority and control over the account. The signers listed on this form must match the names listed on the account Acceptance Form. This form should not be used to add or update corporate signers. It must be used in conjunction with an Acceptance Form to ensure we collect all required information about the signers.
